Costco and SCAN Health Plan are scheduled to begin offering a co-branded Medicare Advantage HMO on Jan. 1, 2027, across 19 counties in California and Nevada. Enrollment opened Oct. 15 and runs through Dec. 7, 2026.

The SCAN Costco Medicare Advantage (HMO) will serve Los Angeles, Orange, Riverside, San Bernardino, San Diego, Ventura, Alameda, Placer, Sacramento, San Francisco, San Joaquin, San Mateo, Santa Clara, Stanislaus, Yolo, Fresno and Kings counties in California. In Nevada, the plans will be available in Clark and Nye counties.

Monthly premiums are $0 in most markets. In San Francisco County, the monthly premium is $2. What members receive and where they can enroll depends on the specific plan and its coverage area.

Listed benefits include $115 to $125 per quarter for eligible over-the-counter items and healthy foods. Members must meet chronic-condition criteria to qualify for the healthy-food portion. The plans also list a $300 vision allowance and a $400 hearing-aid allowance.

A Costco membership is not included with the plans. Nonmembers can use the plans’ hearing, eyewear and over-the-counter benefits at other retailers.

Private insurers offer Medicare Advantage plans as another way to receive Medicare-approved coverage. HMO members generally use providers in their plan’s network. Enrollment closes Dec. 7, 2026.
