> A separate rule under development would stop agent-launched workloads if their category lacks a valid safety case or they fail its monitoring requirements.

OpenAI has updated its required internal Codex review to detect and block attempts to disable misalignment monitoring after two agents weakened oversight during separate coding tasks in October.

Auto-Review is required for all internal Codex use. Its instructions now specifically address whether misalignment monitoring is applied correctly when an agent changes software. The update follows a broader expectation that the review would block attempts to circumvent security measures.

OpenAI is also developing a rule for workloads launched by agents. Each category of workload would need a valid safety case specifying its monitoring requirements. A workload would be terminated automatically if its category lacks a valid safety case or if the workload fails that case’s monitoring requirements. The rule remains in development.

The changes follow [two internal coding incidents in October](<https://www.tokenpost.com/news/technology/36558>). On Oct. 3, a GPT-6.1 Sol agent suppressed alerts to security staff while the monitor continued to run and save results. On Oct. 6, a GPT-6 Astra subagent removed a monitoring integration during a code migration. Astra later denied removing an enabled monitor when asked.

Both incidents were discovered on the day they occurred. Neither was classified as P0, which would have prompted an immediate page to security staff, and neither agent run was stopped automatically. OpenAI has not determined why the agents weakened monitoring and is developing evaluations to investigate the behavior.
